In 2015, Thushani joined Albury Wodonga Health and made it her home, embracing the joys of country living. She completed her Advance Diploma in Obstetrics as well as her general practice training locally in Wodonga. Thushani's professional satisfaction lies in overseeing pregnancies and providing care to families with young children. Often patients do not understand what bulk billing actually is.It is not a discount from the government. If a doctor chooses to bulk bill, the government pays the doctor the Medicare rebate direct and the doctor cannot charge any more. Doctors are then also excluded from charging for consumables they may also have used in the course of the consultation, as this is part of the terms of accepting a bulk bill consignment.Bulk Billing is in fact a direct discount from the doctor, a reduction in his earnings, and the income to the practice to cover running costs. So those patients that say bulk billing is their right, it is not. It is the right of the doctor to decide if they wish to take a devalued amount for their service. Since its inception in 1984, the Medicare rebate for a standard consultation has not kept up with inflation. In fact, from 2014 to 2020 the government actually froze this rebate. Even the last rise was only 65 cents, an increase of 1.6% when inflation was 5.1% for the same period. If the rebate had kept pace with inflation it would now be $80 whereas it is only $39.75.The cost of running a practice has increased significantly, and the rebate is no longer viable to continue running a General Practice. This has also caused a disincentive for doctors to take up a General Practice pathway, so less new doctors are taking up careers as GPs. All this puts pressure on supplying good clinical services for a general practice.
From July 2023 there may be fees for particular nurse consultations.
